If an applicant meets the preceding eligibility criteria, he/she must obtain an Application Form from the high school guidance counselor, from the Scholarship Fund or from this web site www.faryscholarship.org Each applicant must submit:
1) Fary Memorial Scholarship Application Form;
2) High school transcript and recent SAT or ACT test scores;
3) Three Letters of Recommendation must include either (a) two letters from current or former high school academic instructors OR (b) one letter from a high school guidance counselor, high school appointed advisor, or high school principal and one letter from a current or former high school academic instructor. The remaining Letter of Recommendation may be from any other person of the applicant's choice;
4) Financial aid for each college/university being considered, and pending scholarship applications, if any; and
5) Any other information which might be helpful to the scholarship committees.
The completed Application Form, Letters of Recommendation and supporting documents must be mailed (postmarked) or delivered by hand to the Scholarship Fund in ONE package not later than December 15, 2011. The first semester/trimester, senior year transcript must be mailed (postmarked) or delivered by hand not later than February 17, 2012.
